29 CFR 1910.146(d)(5)(i): Conditions in the permit space were not tested to determine if acceptable entry conditions existed before entry was authorized to begin: On or about March 2, 2023, the employer did not ensure atmospheric testing and monitoring was performed to evaluate the conditions in permit spaces, such as the #4029 Mississippi Tank Company manufactured tanker trailer. Employees entering the trailer without atmospheric testing and monitoring were exposed to atmospheric hazards, such as oxygen deficiency and flammable gasses. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
