29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions. (a) On or about February 28, 2023, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Center Bay, the Ogden Dart 2101, S/N: 2156 12-30-09 was utilized with two Lincoln Arc Welders 287007 and 286020 used to weld various components had electrical tape wrapped around the cable due to damaged insulation exposing employees to electrical shock hazards. (b) On or about February 28, 2023, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Center Bay, the Ogden Dart 2101, S/N: 2156 12-30-09 was utilized with two Lincoln Arc Welders 287007 and 286020 used to weld various components had exposed copper wires from the grounding leads exposing employees to electrical shock hazards. (c) During the week of February 20, 2023 and thereafter, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Bridge Bay, the Lincoln Squirt Welder #1 used to weld various components was rewired and exposing the wire and wrapped with electrical tape exposing employees to electrical shock hazards.
