29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Facility-Wide - The employer failed to ensure that an evaluation of each powered industrial truck operator's performance was conducted at least once every three years, on or about February 28, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
