29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) This violation occurred on or about February 24, 2023, where employees doing maintenance work on machines in the converting production department and did not apply lockout/tagout devices to the Mercury 3 machine, serial number 20201201. Employees were exposed to a workplace hazard. CARTAMUNDI DALLAS, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1910. 147 (d)(4)(i) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1595720, CITATION NUMBER 1, ITEM 2D THAT WAS ISSUED ON SEPTEMBER 6, 2022, WITH RESPECT TO A WORKPLACE LOCATED AT 5101 HIGHLAND PLACE DRIVE, DALLAS, TX 75236.
