29 CFR 1910.146(d)(2): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not identify and evaluate the hazards of permit spaces before employees entered: The employer was failing to protect employee from the hazards of entering a confined space. This was most recently documented on February 16, 2023 at 417 West Railway, Bruning, Nebraska. Employees were cleaning and entering boot pit(s) without evaluating the space prior to entry.
