29 CFR 1910.119(j)(4)(iv):The employer did not document each inspection and test that has been performed on process equipment. The documentation shall identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test. IVEX Protective Packaging located at 440 Stolle Avenue in Sidney, Ohio 45365: On or about February 14, 2023 the employer did not ensure documentation of inspections on process equipment, such as but not limited to the oxidizer fan, gas train, ductwork and piping for the extrusion line, included the date of the inspection, name of the person performing the inspection, equipment identifier, and the results of the inspection. Employees were exposed to isobutane hazards such as, but not limited to, frostbite, deflagration, and explosion.
