29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Dakota Digital Inc. at 4510 West 61st Street North, Sioux Falls, SD 57107: On or about and at times prior to February 9, 2023, the employer did not ensure the Color Inkjet Printer 1 in the Production area closest to the Laser room Model: JFX200-2513EX, Serial Number: CJ141136 was properly guarded. This condition exposed employees to caught-in and amputation hazards associated with approximately 3 foot 5-inch area of unguarded mechanical action of the print head as it moved about the table. (b) Dakota Digital Inc. at 4510 West 61st Street North, Sioux Falls, SD 57107: On or about and at times prior to February 9, 2023, the employer did not ensure the Color Inkjet Printer 2 northwest of Printer 1 in the Production area was properly guarded. This condition exposed employees to caught-in and amputation hazards associated with approximately 3 foot 5-inch area of unguarded mechanical action of the print head as it moved about the table. (c) Dakota Digital Inc. at 4510 West 61st Street North, Sioux Falls, SD 57107: On or about and at times prior to February 9, 2023, the employer did not ensure the Color Inkjet Printer 3 directly north of Printer 1 in the production area was properly guarded. This condition exposed employees to caught-in and amputation hazards associated with approximately 3 foot 5-inch area of unguarded mechanical action of the print head as it moved about the table.
