29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: Axomatic Fill Machine #4 Serial Number 04031000M2001: On or about November 10, 2022 through January 25, 2023, operator(s) who perform service and maintenance tasks on the Axomatic Fill Machine #4, Serial Number 04031000M2001 were not adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up, energization, and/or release of stored energy which result in amputation/crushing while cleaning the tube holder and fold jaws, crimp jaws, and flatten jaws. Abatement documentation is required for this item in accordance with requirements of 29 CFR 1903.19(d).
