29 CFR 1910.146(c)(1):The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: At the worksite located at 4747 East Mustard Way, Springfield MO, employees were exposed to caught in and crushed by hazards when the employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces. Employee were entering the cardboard compactor chute to clear jams. This was documented on or about 1-11-2023.
