29 CFR 1910.119(f)(1)(i)(D): The employer did not develop and implement written operating procedures that provide clear instructions for emergency shutdown, including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner. Establishment: On or about December 19, 2022, the employer did not implement effective emergency shut down procedures of the ammonia refrigeration equipment in that the operating procedures did not provide clear instructions for shutdown of ammonia refrigeration equipment in the event of an uncontrolled release.
