29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s) On or about December 21, 2022, the employer failed to ensure that the energy isolating devices of the Line 2 Ironer were located and operated to isolate the energy sources. An employee opened the bridge of the machine, thus entering the hazardous area of the equipment while it was still in operation, in order to place a can inside to test the vacuum suction component of the machine. The safety interlock relay switch was not functional at the time. The employee was exposed to struck by and or caught in between hazards and hazardous energy sources including but not limited to electrical and pneumatic.
