29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: A) On or about November 30, 2022, an employee was working in a trench box located in a 12-foot-deep trench and was not protected by the use of a hardhat. B) On or about December 5, 2022, an employee was working in a 16-foot-deep excavation and was not protected by a use of a hardhat.
