29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (a) On or about December 10, 2022, employees were performing servicing and maintenance on a lift table comprised of a roller conveyor and LS Series lift table by Southworth. Hardware was not provided to secure and/or block the upper portion of the lift table loaded with the roller conveyor.
