29 CFR 1910.119(l)(1): The employer did not establish written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process. a) PHRT refinery, St Croix, VI 00820 - Dimersol Unit- On or about November 14, 2022, changes to the relief system on the Ammonia Storage Vessel (D-7305) including but not limited to isolating and/or blocking the relief path of the ammonia storage vessel's relief devices to the flare and/or a safe location, were not evaluated and documented through the management of change process . Failure to provide an unobstructed relief device path to a safe location could lead to an over pressure of the ammonia storage vessel and employee exposure to anhydrous ammonia. b) PHRT refinery, St Croix, VI 00820 - #3 LPG Fractionation Unit - On or about November 14, 2022, changes to the relief system of the #3 fractionation unit including but not limited to isolating and/or blocking the relief path of the LPG vessel's relief devices to flare and or/a safe location, were not evaluated and documented as part of the management of change process. Failure to provide a relief device path to a safe location could lead to an over pressure of the LPG process vessels and/or equipment and exposing employees to fire and explosion hazards.
