29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a-54-58 Hearthstone Court, Mahwah NJ 07430: An employee engaged in roofing activities using a portable extension ladder for access was exposed to fall hazards of approximately 30ft in which the employee did not keep 3 points of contact while accessing the ladder. Condition observed on or about 11/15/2022.
