29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) are not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished: Cup Making Department: On or about October 14, 2022 employee was exposed to machine hazards from moving parts and burns when employee was engaged in servicing and maintenance activities on the Cup Making Machine #2, Line #9. The employer does not ensure that energy control application steps are implemented prior to allowing employees to clean, or remove accumulated cup residue. The machine is not shut down or turned off prior to engaging in these activities, exposing employees to thermal hazards that could result in severe burns. The employee was not protected from the hazardous energy of machinery or equipment and suffered third degree burns to the right hand. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
