29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a.) Warehouse Freezer - On or about September 9, 2022, an employee used the Raymond Reach Forklift, Model 540-0P03OTT, Serial No. 540-11-A11421, to restack boxes on a rack in aisle 13/14 of the freezer. The employer failed to ensure that employees who operate powered industrial trucks are evaluated at least once every three years.
