29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about August 24, 2022, employees at MiTek Inc. were exposed to point of operation hazards. An Accurpress 100 Ton Press Brake, Model #710010 / Serial #6657, did not have a guarding device designed or constructed to prevent the operator from having any part of his body in the danger zone during the operating cycle.
