29 CFR 1910.146(d)(6): The employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations: (a)On 8/15/2022 and at times earlier, in the Coke Hopper, contractor employees were exposed to falls of up to 13 feet to the bottom of the hopper, and hazardous energy at the point of operation for the vibrator because CORE did not provide at least one attendant outside the hopper into which entry was authorized for the duration of entry operations.
