29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about August 8, 2022, set-up employee(s) did not receive training in the function of the energy control program, and the safe application, usage and removal of energy controls for hazardous energy sources. Employee(s) were exposed to amputation and crushing hazards while performing servicing and maintenance on the BuhlerPrince die cast machine #61 (MN 836CCA, SN DB7004). Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
