29 CFR 1910.119(j)(5):The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use On or about August 5, 2022 in the Amines Ethanolamine unit, employees were exposed to hot triethanolamine and diethanolamine when a process release from the casing drain of Pump G2416 was not corrected and the area was not barricaded to ensure safe operation while this deficiency in the equipment was present.
