Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) For the cooler room, on or about 08/05/2022 and at times prior to, the employer exposed employees to potential amputation and electrical hazards, in that, push buttons and manual motor starter circuits were used to de-energize chain drive motors for maintenance/servicing work rather than circuit breakers which were located in the MCC room. Maintenance/servicing work consisted of activities such as, but not limited to, changing motors, belts, chains and sprockets. The motor starter circuits and push buttons were not designed to be used as energy isolating devices for lockout tagout.
