29 CFR 1926.651(j)(2):Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about July 25, an employee was exposed to struck by hazards while working in a trench where the spoil pile was not kept at least two feet from the trench edge. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
