29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: At a worksite located at 800 Atlantic Street, Kansas City, Missouri. Employees were exposed to serious struck by injuries from the Yale Forklift serial # A390V12530T, that was in operation, in that the brake pad was found to be missing, and in need of repair.
