Tonnard Manufacturing Corporation, Bay 3 press area, on or about 07/06/2022 and times prior thereto: Employees performed servicing and maintenance work on equipment/machinery that included, but were not limited to, machine #300 Pacific 250 Ton Hydraulic Press. The equipment/machinery was not locked out during the die/tooling change setup.
