29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Fabrication Department: Employees did not utilize lockout tagout procedures when clearing jammed pieces of metal from inside of the Amada LCG 3015, observed on 6/9/22. b) Fabrication Department: Employees did not utilize lockout tagout procedures when removing fabricated pieces of metal from inside of the Amada ENSIS AJ Series Laser Cutting Machine, observed on 7/12/22. c) Fabrication Department: Employees did not utilize lockout tagout procedures when cleaning and servicing the Timesavers Incorporated Series 22 sanding machine, on or about 6/9/22. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
