29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a. On or about May 13, 2022, at the facility, employees were changing-out rotary die cutting boards on a rotary die cutter machine, during which time the employer failed to ensure that a periodic inspection of the energy control procedure had been conducted at least annually to ensure that the procedure and the requirement of this standard were being followed; therefore, exposing the employees to amputation hazards.
