29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about May 5, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the pinch points and/or in-going nip points of moving and/or rotating parts likely to result in serious injury. Operators also had access to unguarded danger zones of moving mechanical parts such as, but not limited to die rams, rotating shafts, universals, rockers, clamps, and hold downs on: Vertical hydraulic broach machines (21b6, 21b9, 21b11), threading machines (342t1, 342t2, 342t4, 342t6, ), varnishers (342t7), and multi-spindle drilling head machines (D1,2,4,5,6,7) b) On or about April 26, 2022, the employer did not ensure that operators assigned to hand-feed materials into unguarded points-of-operation were protected from the pinch points and/or in-going nip points of moving and/or rotating parts likely to result in serious injury. Operators also had access to unguarded danger zones of moving mechanical parts such as, but not limited to die rams, drive shafts, universals, rockers, clamps, and hold downs on: Brother CNC machine center 22TC8, the interlock mechanisms did not function to prohibit the machine from cycling while an employee had the front door or inner door open. c) On or about May 5, 2022, the employer did not ensure that operators assigned to operate automated (robotic equipment) production cells were protected from the movement of the robot itself and accompanying machine components. Employees are able to and do enter the robot cell area. Automated centerless grinder (g9 and machine 17 ), Automated CNC machine #14109 (robot equipped).
