29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS) (IDENTIFY MACHINERY OR EQUIPMENT) (WHAT SPECIFIC REQUIREMENT(S) WERE LACKING) (a) This violation occurred on or about April 29, 2022, and times prior thereto, where the each authorized employee did not affix his/her personal lockout or tagout device. Employees were exposed to an amputation hazard.
