29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Fermentationexperts, Inc. at 1201 North Ellis Road, Sioux Falls, SD 57107: On or about and at times prior to March 23, 2022, the employer did not ensure that lockout tagout procedures were established and utilized during operations on the rotating valve where an employee suffered amputation injuries. This condition exposed employees to amputation and caught-in hazards associated with the rotating valve. (b) Fermentationexperts, Inc. at 1201 North Ellis Road, Sioux Falls, SD 57107: On or about and at times prior to March 24, 2022, the employer did not ensure that lockout tagout procedures were established and utilized during operations on the cookie wagon/grinder. This condition exposed employees to amputation, caught-in, and electrical hazards associated with the machine.
