29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Sheetmetal Area: The roll bending machine was not equipped with a safeguard exposing employees to a potential caught in hazard, on or about March 15, 2022. b) Machine Shop: Three horizontal metal lathes (Poreba, Monarch and Sigma) were not equipped with chuck guards exposing employees to a struck by hazard, on or about March 15, 2022. c) Sheetmetal Area: The two Cincinnati press brakes and the Lynch press brake were not equipped with safeguards exposing employees to a potential amputation hazard, on or about March 15, 2022. d) Sheetmetal Area: The blade guard on the Cincinnati shear was raised too high and had openings which would permit employee access to the cutting blade exposing them to an amputation hazard, on or about March 15, 2022.
