29 CFR 1910.119(d)(3)(ii):The employer did not document that equipment complies with recognized and generally accepted good engineering practices. (a) On or about March 1, 2022 and at times prior thereto at the Pioneer Frozen Foods facility in Duncanville, Texas, employees were exposed to ammonia chemical hazards and other chemical hazards when the employer did not ensure the ammonia refrigeration system complied with its selected RAGAGEP (IIAR Bulletin 109) when it did not site and ensure an operable eyewash/safety shower immediately outside the door to the engine room. (b) On or about March 1, 2022 and at times prior thereto at the Pioneer Frozen Foods facility in Duncanville, Texas, employees were exposed to ammonia chemical hazards and other chemical hazards when the employer did not ensure that engine room ventilation system design compiled with its selected RAGAGEP for machine room ventilation airflow rate, machine room temperature control, and air changes per hour. (c) On or about March 1, 2022 and at times prior thereto at the Pioneer Frozen Foods facility in Duncanville, Texas, employees were exposed to ammonia chemical hazards and other chemical hazards when the employer did not document that Pressure Relief Valve(s) comply with RAGAGEP when the employer left them in service for more than five years. (d) On or about March 1, 2022 and at times prior thereto at the Pioneer Frozen Foods facility in Duncanville, Texas, employees were exposed to tripping hazards and electrical hazards due to installation of the temporary wiring in the engine room that did not meet the employer's electrical specifications for installation (RAGAGEP).
