Located at 9295 Dewitt Drive in Plain City, Ohio: On or about March 3, 2022, the employer did not ensure that employees performing roofing work were protected from falls by the use of a personal fall arrest system, thereby exposing employees to fall hazards from approximately twenty feet. Gilberto Miramontes DBA Miramontes Roofing was previously cited for a repeat violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1462135, citation number 2, item number 3a and was affirmed as a final order on 06/08/2020, with respect to a workplace located at 834 Cranberry Drive, Pataskala, Ohio. Gilberto Miramontes was previously cited for a repeat violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1221314, citation number 2, item number 2 and was affirmed as a final order on 06/07/2017, with respect to a workplace located at 8731 Bobwhite Drive, Lot 57, Blacklick, Ohio. Gilberto Miramontes was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1207544, citation number 1, item number 2 and was affirmed as a final order on 04/10/2017, with respect to a workplace located at Lot 33, Celtic Crossing, Dublin, Ohio.
