29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Williams-Sonoma Stores, Inc. at 5503 Havana Street in Denver, Colorado: On or about February 24, 2022, and at times prior, the employer did not ensure that temporary workers who operated standup forklifts were trained in safe operation of the equipment. This condition exposed employees to struck-by hazards and temporary workers to fall and crushing hazards.
