29 CFR 1910.147(f)(3)(i):When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device. a. On or about February 22, 2022, the employer failed to maintain a lockout/tagout (LOTO) program that gave specific procedural steps for shutting down, isolating, blocking, and securing the mobile four-inch pump to control hazardous energy. This LOTO failure resulted in an employee been hospitalized due to burns to the upper torso and arms.
