29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Worksite, 211 North Main Street, Stillwater, New York, Roof of Facility: On or about February 09, 2022, an employee was using two retractable lifelines daisy chained together and connected to a horizontal lifeline system while working from the roof of a commercial building approximately 36-feet above the ground. The retractable lifelines and the horizontal lifeline system did not limit free fall distance to 6-feet. b) Worksite, 211 North Main Street, Stillwater, New York, Roof of Facility: On or about February 09, 2022, an employee was using a personal fall arrest system from the roof of a commercial building approximately 36-feet above the ground. The employee was tied off approximately 50-feet from the anchor point and approximately 10-feet from the leading edge exposing the employee to a free fall of more than 6-feet. c) Worksite, 211 North Main Street, Stillwater, New York, Roof of Facility: On or about February 09, 2022, an employee was using a personal fall arrest system from the roof of a commercial building approximately 36-feet above the ground. The employee was tied off approximately 65-feet from the anchor point and approximately 10-feet from the leading edge exposing the employee to a free fall of more than 6-feet.
