29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) De-bagger #2; On or about January 20, 2022 employees were exposed to amputation hazards in that an employee was allowed to conduct maintenance on the #2 De-bagger without first utilizing the LOTO procedure. b) Palletizer; On or about January 27, 2022 employees were exposed to amputation hazards in that employees were allowed to conduct maintenance activities such as cleaning/jam clearing without first utilizing the LOTO procedure.
