29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards related to facility siting: On or about January 05, 2022, KOCH and contract employees engaged in supporting, contracting, operating, inspecting, testing, and maintaining activities at the covered process equipment throughout the plant were exposed to catastrophic release of ammonia which results in asphyxiation, chemical burns, explosion and flash fire leading to death and injury to persons in and near the facility in that the process hazard analysis did not address the hazards related to the facility power system. The current process hazard analysis report and all of the previous reports did not include any provision, analysis, description, procedure, study or scenario discussing about power outage resulting in the alarms did not sound, the evacuation sirens did not audibly alarm; the security ID badge scanning not working; the egress route mechanism not operating as designed, and the egress route turnstile malfunctioning. There was no signage explaining, showing additional evacuation locations in the event of chained, shut, blocked exits, or egress route turnstile malfunction for contractors to follow to exit the plant in a safe and ordered manner.
