29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. On or about December 29, 2021, employees were exposed to a fall of approximately 21 feet while performing roofing activities on a residential home located on Lot 27 Emerald Woods, Columbia Station, Ohio 44028. Employees were using an extension ladder to access the roof and the employer did not ensure that it was set at a safe angle.
