29 CFR 1910.147(d)(4)(i): Lockout devices were not affixed to each energy isolating device by authorized employees: a) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A mechanic was exposed to amputation hazards while working on a maintenance issue on an energized Wonton machine, Daehan Machinery Co. Ltd Model DHW-32. Violation was observed on or about December 13, 2021. b) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - The Engineering Manager was exposed to amputation hazards while investigating a maintenance issue with the W.E. Plemons Machinery Services, Inc. Model #3000800 Serial# 18-09-1809 Boxing Machine as he did not have the machine locked out and an unknowing employees started the machine. Violation occurred on or about January 20, 2022. c) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A mechanic was exposed to amputation hazards while working on replacing a door interlock switch as they hung an "Equipment Out Of Service" tag near the local disconnect after shutting it off on the Ilapak Packaging Machinery Serial Number 0642220241 packaging machine. Violation was observed on or about February 17, 2022. d) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A mechanic was exposed to amputation hazards while clearing an obstruction while the Daehyun Hi-Tec Ci LTD, Model # DH33EH s/n 2K11712144 was energized. Violation occurred on February 18, 2022. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
