29 CFR 1910.119(j)(4)(i): The employer did not perform inspections and tests on process equipment. Plant; on or about December 6, 2021, and at times prior thereto, employees were exposed to ammonia inhalation, burn and explosion hazards, in that the employer did not perform inspections and tests on the V2/33, threaded bonnet Henry check valve and similar valves to ensure the equipment was in safe operation condition.
