29 CFR 1910.119(j)(4)(ii): Inspections and testing procedures performed on process equipment to maintain its mechanical integrity did not follow recognized and generally accepted good engineering practices (RAGAGEP): This violation was most recently observed on or prior to October 27, 2021, in the Honeywell G-245 Process Unit located in Geismar, Louisiana when the employer did not ensure that inspections and tests performed on process equipment follow recognized and generally accepted good engineering practices (RAGAGEP) such as Ultrasonic Testing (UT)/material thickness testing, including but not limited to: a) 1" PRBSH08611 (P&ID 574-D-0068-1), b) Recovered HF & Org 1" PJH06218S from P-6112A/B to E-6101D (P&ID 574-D-0068-1), c) Fresh HF 1" PJH06116 from P-6110A/B to E-6101D (P&ID 574-D-0068-1)
