29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly a. 1600 West Blancke Street, Linden, NJ - Production Area The employer did not ensure that attachments were certified by the manufacturer. Employees engaged in the manufacture of resins worked from a company manufactured, personnel cage/basket attached to a Multiton SM20 Walker Stackie forklift, which was not properly documented as being authorized for use by the manufacturer for use on the equipment. Violation observed on or about October 19, 2021
