29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) On or about 10/01/2021, in the loading dock, since 2009 the Employer did not provide a formal training and practical evaluation to employees driving fork lift trucks. Employees used Power Industrial Vehicles (PIVs) such as: Toyota Model 2FBCA15; Serial 2FBOA15-130 to load/unload bakery items. ABATEMENT CERTIFICATION REQUIRED
