29 CFR 1910.252(a)(2)(xiii)(D): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, and did not advise all contractors about flammable materials or hazardous conditions of which they may not be aware: IN THE ALTERNATIVE 29 CFR 1910.119(h)(3)(v): The contract employer did not advise the host employer of any unique hazards presented by the contract employer's work, or of any hazards found by the contract employer's work. (a) On or about September 27, 2021 and at times prior thereto at the DA-101 Quench Tower in the Petro2 Unit, the workers were exposed to fire and explosion hazards when contractors in the area were not informed of hazardous conditions.
