29 CFR 1910.146(i)(5):The employer did not communicate with authorized entrants as necessary to monitor entrant status and to alert entrants of the need to evacuate the space under paragraph (i)(6) of this section; a) Omnisource LLC is located at 1500 Old Bowman Street Road Mansfield, OH, 44903: On or about September 9, 2021, an employee entered a permit-required confined space, Harris Baler Model TG 2102 to perform welding on a baler ram and was not provided with means to communicate his status in case of an emergency. The employer did not communicate with authorized entrants as necessary to monitor the entrant's status when the metal ram dropped approximately 8 inches hitting the entrant in the head and shoulder.
