29 CFR 1910.119(j)(2):The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment. On or about July 27, 2021 at the Lyondell Basell Chemical Manufacturing facility in La Porte, Texas workers were exposed to process materials, toxic release, and flammable hazards from the Acetic Acid Unit when the employer did not establish and implement written procedures to direct the workers in modifying HV-117, which was installed in process but not isolated, so that it could be used as an isolation point in the LOTO of the methanol supply piping to the Acetic Acid Reactor.
