29 CFR 1926.451(a)(6):Scaffolds were not designed by a qualified person: a) Miller Family Construction, LLC, located at site location 1005 Somerset Drive, Perrysburg, Ohio, on or about July 2, 2021: The employer did not ensure the scaffolding platform used to elevate employees via the Skyworks G6 Tele handler forklift, was designed by a qualified person. Employees performing construction activities in this condition were exposed to a fall of approximately 23 feet.
