29 CFR 1910.252(b)(3): Employees exposed to the hazards created by welding, cutting, or brazing operations were not protected by personal protective equipment in accordance with the requirements of 29 CFR 1910.132: On or about June 23, 2021, at the location of 5535 Wegman Rd., Valley City, OH 44280, line 12, die cut area, personal protective equipment such as, but not limited to a welding helmet and flame retardant clothing were not in use while an employee operating a plasma cutter was exposed to fire hazards.
