Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1928.57(a)(11):Servicing and maintenance. Whenever a moving machinery part presents a hazard during servicing or maintenance, the engine shall be stopped, the power source disconnected, and all machine movement stopped before servicing or maintenance is performed, except where the employer can establish that: a. On or before June 28, 2021, the employer did not ensure employees disconnected and locked out the electrical power source for the North Tunnel Belt conveyor while cleaning the conveyor adjustment room and the area between the belt and roller. On June 20, 2021, an employee used the E-stop button when the conveyor restarted, resulting in an amputation of the employee's right arm.
